Insurance

Surbhi is currently out-of-network and does not take insurance.

Working as an out-of-network provider allows for an increased level of confidentiality and scope of treatment.

However, depending on your current health insurance provider and plan, you may be eligible for partial or full reimbursement.

After each session, we can provide you with a receipt detailing the information needed to submit a claim to your insurance.

You can contact your insurance company directly to inquire about reimbursement options.

Below is a list of questions to consider asking your insurance company:

  •    Do I have out-of-network benefits for psychotherapy?

  •    The CPT code for a typical individual psychotherapy session is 90834 or 90837. The code for a typical couples psychotherapy session is 90847

   •    Is prior authorization required?

   •    Do I have an out-of-network deductible and how much of the deductible have I met so far?

  •    How much is the insurance company’s “usual and customary fee” for out-of-network therapy and what percentage do they cover?

  •    Is there a limit to the number of sessions they cover?

Under the "No Suprises Act", each client is entitled to a Good Faith Estimate.  See CMS.gov for more information.
